For your first visit:
WHEN TO ARRIVE
Arrive 20 minutes early to fill out paperwork or you may download and complete forms and arrive 5 minutes prior for check in.
WHAT TO BRING
- Paperwork (if completed ahead of time)
- Insurance Card
- Driver’s License
- Physician’s Prescription and/or post surgical instructions (if applicable)
- Comfortable clothing that allows access to your region to be evaluated
- Co-Pay
- Any questions you have
APPOINTMENTS
Your appointment will last between 30-90 minutes, your therapist will be more specific after your initial evaluation has been completed.
